fa بررسی هزینه‌های ناشی از درخواست معافیت پزشکی به دلیل تشنج در سربازان مراجعه کننده به بیمارستان بعثت نهاجا در سال 1387 تحقیقی Original <div style="text-align: justify direction: rtl"> <span style="font-weight: bold">مقدمه:</span> با توجه به مراجعات فراوان سربازان با ادعای صرع و نادرست بودن بسیاری از این ادعاها بر اساس آیین‌نامه فعلی و از طرفی هزینه‌های تحمیلی ناشی از ارزیابی صحت این ادعاها، ضروری است تا با بکارگیری راه‌کارها و قوانینی جدید از زمان و هزینه اضافی تلف شده کاست. این مطالعه با هدف بررسی هزینه‌های ناشی از درخواست معافیت پزشکی به دلیل تشنج در سربازان مراجعه کننده به بیمارستان بعثت نهاجا در سال 1387 انجام شد. <br><span style="font-weight: bold">روش بررسی:</span> در این مطالعه توصیفی-تحلیلی 369 نفر از سربازانی که طی سال 1387 به کمیسیون نورولوژی با ادعای تشنج از یگان‌های مختلف نیروی هوایی مراجعه کرده بودند، وارد شدند. داده‌های کلیه افراد وارد فرم اطلاعاتی شد و پس از تکمیل در نرم‌افزار SPSS مورد تجزیه و تحلیل قرار گرفت. متغیرهای کمی به صورت میانگین±انحراف معیار و متغیرهای کیفی به صورت درصد بیان شدند. <br><span style="font-weight: bold">یافته‌ها:</span> مجموعاً 640 تست EEG، 52 CT اسکن و 28 MRI انجام گرفت. 8/30% افراد مجموعاً 759 روز در بیمارستان با میانگین 5/1±7/6 روز بستری بودند. مجموعاً 4552 روز خروج از نظام برای 369 نفر وجود داشت. اکثر افراد (2/71%) معاف از رزم شدند و فقط 9/14% آنها معاف دایم شدند. مجموعاً مبلغ 900/361/413 ریال جهت افراد مطالعه هزینه صرف شده بود. <br><span style="font-weight: bold">بحث و نتیجه‌گیری: </span>با توجه به مراجعه بسیار زیاد افراد پس از ورود به خدمت وظیفه با ادعای صرع و تقاضای معافیت، پیشنهاد می‌شود که قانون معافیت دایم بصورت روتین در مورد صرع در زمان صلح برداشته شود و برای مراجعه کنندگان با ادعای بیماری صرع به صورت روتین پس از اثبات، معافیت از رزم داده شود و معافیت دایم فقط در مواردی اعمال شود که فرد عقب ماندگی ذهنی، ضایعه مغزی یا موارد خاص تشنج‌های کنترل نشده و استفاده از 2 یا 3 دارو داشته باشد. </div> <span style="font-weight: bold">Background:</span> As there are lots of cases where conscripts complain of epilepsy and not all the complaints are based on reality, the imposed costs on the medical agents to understand the reality is very high. So it is necessary to consider new laws and strategies to decrease the time and cost of such cases. This study was conducted with the aim of studying the costs of medical exemption due to seizure in conscripts visiting the Be'sat Hospital of IRI Air Force in the year 2007. <br><span style="font-weight: bold">Materials and methods:</span> In this study, 369 conscripts which had applied to the neurology commission with the claim of seizure from the different bases of the Air Force. The information from all the patients was inserted into an information form and after completing, inserted into SPSS software for analysis. The quantitative variables are announced as mean ± SD and qualitative variables are announced in percent. <br><span style="font-weight: bold">Results:</span> In total, 640 EEG tests, 52 CT-scans and 28 MRIs were used. 30.8% of the patients used a total of 759 days in the hospital with an average 6.7±1.5 days in bed. In total the 369 conscripts had been away from their military work for 4552 days. Most of the people (71.2%) were exempted from war and only 14.9% were completely exempted from the military. In total 413, 361, 900 Rials was spent on their medical condition. <br><span style="font-weight: bold">Conclusion:</span> As lots of conscripts claim having epilepsy and seizure so that they can be exempted from military service, it is advised that the law of permanent medical exemption from military service be removed for those complaining of epilepsy in time of peace and only exemption from war be considered for such cases.
